If your whole body aches for no clear reason, if you’re drained by mid-afternoon, or if your feet tingle at night, the explanation may not be a disease at all. It may be two missing nutrients. Study after study finds that a large majority of city-dwelling Indian adults are short of vitamin D, despite all our sunshine, with many more sitting in an “insufficient” zone that gets waved through as almost normal. Vitamin B12 deficiency is common too, especially among vegetarians, long-term metformin users and older adults. And these deficiencies keep telling company: doctors regularly find low vitamin D alongside bone and joint pain, muscle weakness, osteoporosis and poor recovery from exercise. The deficiency is silent. Its consequences are not.
What Vitamin D Actually Does: Far More Than A Bone Vitamin
Most people know vitamin D helps calcium reach the bones. Fewer know that muscle cells carry vitamin D receptors too, meaning docking points where the vitamin plugs in and does its work. When levels fall, muscles become measurably weaker, ache more, and recover more slowly from exercise. Vitamin D also supports the immune system and helps keep inflammation in check. So a deficient person can suffer three problems at once: bones quietly losing density, muscles losing strength, and a body-wide ache that no scan explains. In older adults, especially women past menopause, this triple effect directly accelerates the degenerative conditions BFLL works with: osteoporosis, sarcopenia (the age-related shrinking of muscle) and painful joints.
Why Is Sunny India So Deficient?
It sounds absurd. Kolkata bakes in sunshine for most of the year, yet deficiency is the urban default. The reasons are practical, not mysterious:
- Indoor living. Office, car, home, lift. Many of us go days without direct sun on bare skin, and glass windows block the UVB rays that make vitamin D. Sun enjoyed from behind a window is light, not medicine.
- Clothing and habits. Full-sleeved clothing, sun avoidance and sunscreen all reduce the skin’s production. Sensible for skin protection, costly for vitamin D.
- Skin tone. Melanin, the pigment that gives Indian skin its colour, works like a built-in sunscreen. Darker skin needs considerably longer in the sun to make the same vitamin D as fair skin.
- Pollution. Urban haze filters out part of the UVB before it ever reaches street level, a real factor in metropolitan Kolkata.
Put these together and “sunny country” becomes irrelevant. For city dwellers, deficiency is the rule, not the exception.
Decoding Your Report: Deficient, Insufficient, Sufficient
Vitamin D is reported as 25-hydroxy vitamin D, measured in ng/mL. In plain words: below 20 is deficient, meaning the tank is nearly empty. Between 20 and 30 is insufficient, enough to avoid severe bone disease but not enough for muscles and bones to work well. Above 30 is generally considered sufficient. Think of your phone battery. At 5 percent it’s an emergency. At 25 percent the phone still switches on, but you wouldn’t trust it for the day. That 20-to-30 zone is the 25 percent battery of vitamin D, and it’s exactly where bone pain, weak exercise performance and slow recovery tend to cluster. People in the low 20s are often told they’re “almost normal” and do nothing. Almost normal is not normal.
Vitamin B12: The Nerve Vitamin
B12 keeps your nerves insulated, helps make red blood cells and supports energy production. Picture the plastic coating on an electrical wire. Strip the coating, and the wire short-circuits. B12 maintains that coating on your nerves, which is why deficiency creeps in slowly and then announces itself as tingling or numbness in the feet and hands, burning soles, wobbly balance, low mood, memory fog and fatigue. Three groups run a particular risk in India: vegetarians, because B12 comes almost entirely from animal foods; people on long-term metformin for diabetes, because the medicine reduces B12 absorption; and the elderly, whose stomachs absorb it less efficiently. Notice the overlap. Many people with diabetes tick two of those three boxes at once. Left untreated, B12 deficiency can cause nerve damage that becomes difficult to reverse, which is why screening beats waiting for symptoms.

How much vitamin D do I need per day?

Can I just take the weekly 60,000 IU sachet everyone talks about?
That regimen is common in India and often appropriate, under guidance and with a re-test after the course. Taking it indefinitely without monitoring, or skipping the re-test, is how people stay deficient for years or, rarely, overshoot.
Is my tingling from B12 deficiency or diabetes?
Both cause similar nerve symptoms, and in metformin users they can coexist. Only testing separates them. Treating the wrong one wastes months while the nerves keep suffering.
Will fixing my vitamin D make my joint pain go away?
